Overview
The purpose of the HSV-1 IgM Serum Test is to detect IgM antibodies against Herpes Simplex Virus type 1 in the blood, helping to diagnose a recent or active primary HSV-1 infection. This test aids clinicians in confirming suspected herpesvirus infections, evaluating patients with orolabial lesions or encephalitis of unknown origin, and monitoring pregnant women who lack prior HSV-1 immunity. Early detection through serology enables timely antiviral therapy and informed counseling regarding disease transmission and management.

Sample Collection
No special preparation such as fasting is required before sample collection. Inform the phlebotomist or healthcare provider about any current medications, recent immunoglobulin therapy, or blood transfusions, as these may affect test results.
Method: Venipuncture
Laboratory Analysis
A standard venipuncture will be performed to collect approximately 2 mL of blood from a vein in the arm. The area will be cleaned with an antiseptic wipe before needle insertion. Mild discomfort or a small bruise may occur at the puncture site.
Report Delivery
Apply gentle pressure with a cotton ball or gauze at the puncture site for a few minutes to stop any minor bleeding. A small bandage will be placed over the site. Avoid heavy lifting with the collection arm for the remainder of the day. No specific post-test restrictions are required.
Timeline: Sample receipt: Tuesday / Friday by 9:00 AM. Report: Same day.

Doctor's Notes
Reviewed by Dr Kothari Ramesh Shantilal — MBBS, MD (Microbiology) · Reg. No. 27670
"HSV-1 IgM antibodies indicate a recent or active primary infection with Herpes Simplex Virus type 1. This test is particularly valuable in evaluating pregnant women without prior HSV-1 exposure, patients presenting with orolabial lesions, and cases of suspected herpes encephalitis. A positive IgM result should be interpreted alongside clinical findings and, when possible, confirmed with HSV-1 IgG testing and molecular methods. IgM antibodies typically appear within 1 to 2 weeks of initial infection and may persist for several weeks. Clinicians should note that IgM can occasionally cross-react between HSV-1 and HSV-2, so definitive type-specific diagnosis may require additional testing."

- Samples with visible microbial contamination
Understanding Your Results
Negative (Index < 0.90)
No detectable HSV-1 IgM antibodies. This may indicate no recent or active primary HSV-1 infection. If clinical suspicion is high and symptoms are present, repeat testing in 2 to 4 weeks is recommended as early infection may not have triggered detectable IgM levels.
Equivocal (Index 0.90 - 1.10)
Borderline result that cannot be definitively classified. Repeat testing in 2 to 4 weeks is recommended to determine if antibody levels are rising or declining. Clinical correlation is essential.
Positive (Index > 1.10)
HSV-1 IgM antibodies detected, suggesting a recent or active primary Herpes Simplex Virus type 1 infection. Clinical correlation is necessary. Consider HSV-1 IgG testing to determine prior exposure status and PCR testing of lesion swabs for definitive confirmation.
Consult a healthcare provider if you have a positive or equivocal HSV-1 IgM result and are experiencing symptoms such as painful oral blisters, fever, or neurological signs. Pregnant women with a positive result should seek immediate obstetric consultation. If you have been exposed to someone with confirmed HSV-1, or if you have a weakened immune system, discuss the results with your doctor for appropriate management and antiviral treatment options.
Limitations
- ⚠IgM antibodies alone cannot definitively distinguish between HSV-1 and HSV-2 due to antigenic cross-reactivity
- ⚠A positive IgM result does not confirm the timing of infection with certainty; IgM can be detected during recurrent episodes in some patients
- ⚠This test should not be used as the sole basis for diagnosis; clinical correlation and additional confirmatory testing are recommended
- ⚠Sensitivity may be reduced in immunocompromised patients who may not mount an adequate antibody response
- ⚠Negative results do not rule out HSV-1 infection if tested too early before seroconversion
Risks & Considerations
- ●Minor bruising or soreness at the venipuncture site
- ●Slight risk of infection at the puncture site (extremely rare)
- ●Rare possibility of fainting during or after blood draw
- ●Very small risk of hematoma formation at the collection site
Interfering Factors
- ●Cross-reactivity between HSV-1 and HSV-2 IgM antibodies may produce false-positive results
- ●Recent administration of immunoglobulin therapy or blood transfusion may interfere with antibody detection
- ●Immunosuppressive medications or conditions may reduce antibody production leading to false negatives
- ●Very early infection before seroconversion may yield negative results despite active infection
- ●Presence of heterophilic antibodies may occasionally cause false-positive EIA results
