Overview
The purpose of the Reticulin IgA antibody test is to aid in the diagnosis of celiac disease and dermatitis herpetiformis. The test measures the presence and level of IgA antibodies to reticulin in the blood. These antibodies are produced by the immune system in response to gluten ingestion in susceptible individuals. Detection of these antibodies supports the clinical suspicion of gluten-sensitive enteropathy and helps the clinician decide whether further diagnostic steps, such as a small intestinal biopsy, are required. The test is also sometimes used as a monitoring tool to assess adherence to a gluten-free diet, although serial tTG antibody measurement is more commonly employed for this purpose.

How to Prepare
- Use a Serum Separator Tube (SST) and collect at least 2 mL of whole blood (1 mL minimum).
- Ship the sample refrigerated or frozen.
- Label the sample clearly with patient name and unique identifier.
- Accompany with a duly filled Test Send Out Consent Form (Form 35).

Understanding Your Results
Positive
Supports the diagnosis of celiac disease or dermatitis herpetiformis. A gastroenterologist should be consulted for further evaluation, including small intestinal biopsy.
Negative
Makes celiac disease less likely. If clinical suspicion is high, additional tests such as anti-tTG, EMA, DGP antibodies, and total IgA levels are recommended.
Weakly Positive / Borderline
Interpret with caution. Correlation with other serological markers and clinical symptoms is advised; repeat testing or additional workup may be needed.
Consult a gastroenterologist if you have persistent abdominal pain, bloating, diarrhea, constipation, unexplained weight loss, chronic fatigue, or iron-deficiency anemia. If you have a first-degree relative with celiac disease, also discuss screening.
Limitations
- ⚠Lower sensitivity and specificity compared with anti-tissue transglutaminase (tTG) and anti-endomysial antibody tests.
- ⚠A positive result should be confirmed with a duodenal biopsy for definitive diagnosis.
- ⚠A negative result does not completely exclude celiac disease, especially in IgA-deficient individuals.
